5th metatarsal base fracture is a common injury. We should distinguish 3 zones

·        Zone 1 (pseudo jones fracture) : proximal tubercle. Usually secondary to hindfoot inversion.

·        Zone 2 (Jones fracture) : metaphyseal-diaphyseal junction, involving the 4th-5th metatarsal articulation. Usually secondary to forfoot adduction

·        Zone 3: proximal diaphyseal fracture, distal to the 4th-5th metatarsal articulation. Usually seen in athletes (stress fracture secondary to repetitive microtrauma)

Treatment

Conservative Treatment is sufficient (Foam Walker) for zone 1 fracture.

 Zone 2 and 3 are at higher risk of non-union: if non-operative treatment is considered, a short leg cast is preferred and follow up is needed to control union. In athletic individuals, surgical Treatment is preffered.
